What it really means
The official name for the doctor's statement about your fitness for work (it replaced the old "sick note"). It can say you are "not fit for work" or "may be fit" with changes, such as lighter duties. Needed after more than 7 days off sick.

工作适应性证明（fit note）：病假条的官方名称，可写明你"不适合工作"或"在调整条件下可以工作"（如减轻工作量）。病假超过7天才需要。
